This post sits within a team working collaboratively with Hospiscare’s Clinical Nurse Specialists to deliver an excellent standard of evidence based specialist palliative nursing care and clinical advice for adults registered with a GP in the Seaton and Axminster area. The postholder will contribute to the assessment and review of patients, prioritising their needs and ensuring effective communication is in place to support patient care.
To apply, you will need to demonstrate experience of caring for patients with life limiting conditions and an understanding of the complexities of working with individuals at times of distress and conflict. You must be able to manage a caseload, prioritising workload and time effectively, working autonomously and acting on your own initiative as required. In addition the ability to work and travel within the community setting, participating in the seven day service provision including nights, is crucial.
