Practical advice
In addition to physical and emotional problems, the financial and practical aspects facing patients with terminal illness can be distressing.
Many patients and their families need specialist help in dealing with health and social care agencies whilst receiving Hospiscare's support.
Our Care Managers try to ease their path by offering help and advice that supports people to stay at home where practical.
All the support you need
Our Care Managers work closely with social services and the PCT to ensure patients who are discharged from the hospice are fully supported. Care managers provide guidance and support in the following areas:
- Help with any forms that need filling in
- Guidance through the maze of bureaucracy, offering useful knowledge of local resources
- Help to accessing equipment that is needed at home
- Advice about available grants from statutory and voluntary agencies
- Advice about housing, social care provision and other similar matters
